4Vaccination Records

Vaccines must be given no more than 1 year and no less than 21 days prior to New Zealand arrival. Copies of booklets/passports are required with your booking. Enter date given, or type N/A if not applicable.

5Approval & Legal Agreement

In the event your pet(s) require immediate veterinary attention, you authorise Pethaven Quarantine 2023 Ltd to seek all required care. You agree to pay all resulting charges as per the contract. If the Import Health Standard is not met, you agree for all testing and treatments as directed by MPI.

Contract Agreement

Legal contract between Pethaven Quarantine 2023 Ltd and the owner named on this form covering all pets listed:

Release documentation issued by MPI Inspector once all Import Health Standard conditions are met. Collection confirmed closer to or on date of release.
Visitations are not permitted during the quarantine period in all AKL facilities.
A $200.00 NZD per pet non-refundable deposit is required to secure your booking.
I accept all MPI and 3rd party charges. If non-compliant on arrival, additional costs apply. Full payment required before release. No pets released until full payment received.
I agree to truthfully complete the 'Facts about Pet(s) Chart', disclosing all behavioural and medical information.
If arrival is delayed, Pethaven may not immediately accommodate my pets. I will confirm new dates as soon as possible.
I authorise Pethaven to collect and transport my pet(s) on my behalf from the airport.
All bedding/toys inside transport crates will be incinerated upon arrival per Biosecurity New Zealand requirements. Personal items returned "as is".
Travel crates must be collected at time of pet release. Disposal fees: $40.00 NZD (wooden) / $30.00 NZD (plastic) for uncollected crates.
Pethaven does not accept responsibility for death, illness, injury or loss for any reason whatsoever.
